To join the Fund follow these instructions:

1. Complete the two attached forms

Application for membership

Payroll authorization card and deliver to City Payroll Office located third floor City Hall or mail to our office and we will mail it to payroll.

2. Check your payroll deductions two payrolls beyond your application date to assure your membership.

3. Always remember to change your beneficiaries in the event of deaths, divorces, etc. Likewise let our office know if your have a change of address.
